The vast majority of visits provided by home health (HH) clinicians are for routine treatment and/or observation and assessment. Achieving consistency in documenting skilled interventions during these visits will have a significant impact on the justification of services provided and the receipt of appropriate reimbursement for those services. Obviously, the goal of any agency is to be paid for ALL the services provided by their clinicians. Clinicians hold the key in this responsibility as each and every clinician must meet the skilled standard each and every visit. Meeting - or not meeting - the standard is determined by how a clinician documents.
Most of us have been providing HH visits for a number of years and many of us come from a time when simply having a clinical note met the standard. But things are very different now and that is simply no longer the case. Old habits are generally hard to break and most of us need a little help to change a pattern that has worked in the past. The significance of our documentation skills simply can't be over emphasized so we ask that you listen carefully to the information you are about to hear and then do a self assessment by taking time to look closely at the contents of your documentation.
