Chronic diseases and conditions cause major limitations in daily living for almost one out of every ten Americans. According to the Centers for Disease Control and Prevention the top five chronic diseases leading to death and disability are:

  • heart disease
  • cancer
  • stroke
  • diabetes
  • arthritis
