Day Care and School Surveys
The Day Care and School Surveys measure the number of up-to-date Certificates of Immunization (IMM-50), or blue slips. All children in Alabama are required to have an up-to-date blue slip on file with their present day care or school. Each day care and school should be visited by a public health representative every four years for education and consultation.
Day Care Surveys
The Day Care Self Survey is conducted October through March each year. The Day Care Validation Survey is conducted March through July each year.
School Surveys
The School Entry Survey is conducted September through November each year. The School Validation Survey is conducted January through May each year.
